NOTICE TO OUR CIGNA PATIENTS

After much consideration, we have made a difficult business decision that may have an impact on your physician-patient relationship. Unfortunately, our renegotiation with Cigna Health Plan has not had a positive conclusion.
North Scottsdale Family Medicine has given Cigna Health Plan notice of termination to be effective January 7, 2008. We have requested confirmation of this date from Cigna; however, they have not provided that verification as of the date of this notice. Therefore, the effective date could possibly be different than the date we have calculated. Per our Agreement with Cigna, we are obligated to provide 60-day advance notice which we gave on November 7, 2007.
Please be assured that we will work with you to ensure that your health care needs are met. It may be possible for your physician to continue caring for you if you have out-of-network benefits; however, it may require more out-of-pocket expense for you. In addition, services are payable on the date they are rendered.
You should be able to determine what kind of impact this choice would mean for you by looking at your Benefits Manual, or by discussing this with your Employer’s Benefits Administrator. You may also have the option of switching to a different health plan that we participate with. However, if the best choice for you is to change doctors, please have your new physician send a medical record request to us and we will send a copy of your medical record to your physician at no charge.
